When a card authorization form makes sense

Any business that works by appointment loses money when a booked slot stays empty: hair and nail salons, barbershops, spas, auto repair shops, tutors, photographers, fitness studios, consultants. A card on file with a signed authorization lets you charge the no-show or late cancellation fee you already announced, instead of chasing the customer afterwards.

It is also useful for deposits on long or expensive services, for monthly memberships or packages paid automatically, and for one-off charges you take over the phone.

How to use the form well

  • Match your public policy. The fees and notice period on the form should be the same ones customers see when they book.
  • Collect only what you need. If your terminal already stores the card, keep the last 4 digits option and leave the security code off.
  • Get it back securely. In person or through a secure upload, never by email or text message.
  • Store it and then destroy it. Keep signed forms locked away and shred them when the authorization is no longer needed.

The paperless alternative: deposits at booking

What is a credit card authorization form?

It is a signed document in which a customer gives your business permission to charge their card under specific conditions: a no-show or late cancellation fee, a deposit, a recurring payment or a one-time charge. It records what was agreed, when and by whom, which helps if a charge is ever questioned.

Does this tool collect or store card numbers?

No. The generator only builds a blank form. There is no field on this page for card numbers, expiration dates or security codes; those appear as empty lines in the document that your customer fills in by hand. Your business details and settings stay in your browser.

Can I use it to charge a no-show or late cancellation fee?

Yes. Choose “Card on file”, enter your no-show fee, your late cancellation fee and how many hours of notice you need. The form spells out those conditions next to the customer's signature. Make sure the same policy appears where customers book, so the fee never comes as a surprise.

Should I ask for the full card number or only the last 4 digits?

If the card is already stored in your payment terminal or processor, the last 4 digits are enough to identify it, and the paper form carries far less risk. Ask for the full number only if you plan to key the payment in by hand, and then treat the signed form as a sensitive document.

How should I store signed authorization forms?

Never ask customers to email full card numbers, and do not send them by email or chat yourself. Keep paper forms in a locked place that only authorized staff can open, keep digital copies out of shared folders, black out or destroy the security code once the payment is authorized, and shred forms when you no longer need them. Follow your payment processor's instructions and the PCI DSS guidance for card data; this tool does not replace legal or compliance advice.

How do I turn the form into a PDF?
